What should I expect?

You’re in great hands. I make sure that all new clients feel welcome, safe, and taken care of. During your first visit, I will review your medical history and perform an initial evaluation to safely and effectively guide our treatment. It is typical to spend up to 30 minutes or more of the first session performing an initial evaluation with the remainder of the session spent on initiating treatment.

To maximize treatment time during your first session, please fill out the new client intake and consents provided on RMTP.Jane.com (when you schedule) before your visit.


"Can I use my Insurance?"

 

No, I do not bill insurance.  I made this choice because I have found that the insurance companies often act as a barrier between my clients and their maximum potential. Also, many clinics required me to dictate portions of treatments to rehab aids or overlap patients. If you would like to use your insurance for physical therapy treatment, there are providers in the area that you can check in with. Most will require a physician's referral to submit to your insurance company and some will charge out of pocket for the trigger point dry needling services. Cost

 

One on one Physical Therapy Treatments (and Evaluations) are as follows:

At our clinic, Carbondale, CO: Evaluation: $220, Treatment: $180/1 hr, $250/1.5 hrs, $320/2 hrs

Offsite within the Town of Carbondale, CO: Based on availability. Base price as above plus $75 travel and set up fee.

Offsite outside of Carbondale, CO: I am happy to accommodate this based on availability. Please contact me for a quote. 

Private BFR Sessions: $65 for one limb (Unilateral), $90 for 2 (Bilateral) Currently unavailable

Cancelation Fee: A missed appointment without 24 hours notice will be charged the full price of the session.

Discounts: We offer a $15/session discount based on financial need for the following populations. If you qualify and feel that you have a financial need for payment assistance, please request this at the beginning of your session. Unfortunately, we are unable to offer these discounts for individuals requesting a superbill for any reason, including submitting for reimbursement from insurance.

  • Students, Teachers, School Social Workers/Counselors, and School Nurses (K-12)

  • First Responders (EMTs, Paramedics, Firefighters, Law Enforcement)

  • Senior Citizens (age 65 and over) with financial need

  • Acute post op sessions requiring a frequency of more than 1/week.

Gift Certificates: May be purchased online. You must sign into your account online to purchase whether it is for you or someone else. After purchasing the gift certificate, please retain the gift certificate number in order to use it in future purchases. Anyone with the gift certificate number may use it, so the same gift card may be shared by a couple, a family, a tribe, etc.

By law, you are entitled to a “Good Faith Estimate”. The prices above are valid as long as they are on the website, this is regardless of your diagnosis or what types of treatment you have. The prices are also listed in the scheduling site when you book. Feel free to take a screenshot of the booking if you would like documentation of your estimated price. If you would like a personalized good faith estimate, please ask your provider. If you are billed for more than this Good Faith Estimate, you have the right to dispute the bill. If you feel that your bill does not match your estimate, please contact Dr. Tanice at RMTP and let her know your concerns. You can ask her to update the bill to match the Good Faith Estimate or ask to negotiate the bill. You may also start a dispute resolution process with the U.S. Department of Health and Human Services (HHS). If you choose to use the dispute resolution process, you must start the dispute process within 120 calendar days (about 4 months) of the date on the original bill. There is a $25 fee to use the dispute process. If the agency reviewing your dispute agrees with you, you will have to pay the price on this Good Faith Estimate. If the agency disagrees with you and agrees with the health care provider or facility, you will have to pay the higher amount.
