Every new client will fill out all of the above forms (4 total). You can either email the completed forms to rockymountaintherapros@gmail.com or print them and bring them in completed to your first appointment. If you are unable to do this, I am happy to provide you with the forms. Please let me know that this is your preference so that I may accommodate the paperwork time in the schedule.
